How is this weighted down so that it continues to hang vertically after repeated opening and closing of the door? Is it just filled with foam?

It attaches to the door with a Velcro strap. The device is filled with insulated foam. They are incredibly durable.

How does the hook and loop set-up work? How is draft blocker affixed to door or window? Many thanks!

You remove the 4 velcro tabs. Using scissors, cut to the correct width of door or window casing. Replace the 4 velcro tabs from the first step. Make sure to evenly place tabs along the full length of the draft blocker. Horizontally align the draft blocker against bottom surface of door or window casing. Carefully remove the filmcovering the velcro tabs. Note: Once cut, this item is non-returnable.

When I opened the delivery box I did not expect to see what I did. A plain piece of foam wrapped in a cloth covering with some Velcro strips. I thought, "This will never work". So I followed the directions, attached the Velcro to the blocker and attached to my doors. Unbelievable. Stopped the drafts coming under the doors and I could still open and close the doors without removing the blocker. My next thought was that I could make these myself. Looked pretty simple. But for the inexpensive price it was not worth it to do myself. Thank you to the person that thought of these!!

I've been super happy with this, though I was a little uncertain at first since it didn't provide as many velcro patches as I would have liked to attach to the door. It still drastically improved the heat retention of the space I got it for, blocking the worst of the freezing air out and I'm even impressed with how much cooler the same space is during the summer heat despite not having any windows or moving air in there. Worth It to buy this!
