Does this wrap have a tie belt or some other fastener? What's the actual size of the "pad"? How would it be used by a person with a waist smaller than 28"? Thanks!

This wrap has a velcro strap to go around the front of the body and hold it in place. The pad section is approximately 13"x11". Unfortunately, it won't accommodate less than a 28" waist.
