The pulley I bought for my broken shoulder rehab goes over the door. It hasa round end to keep it from slipping out when in use I am now in the second week of my rehab The therapist gave me an exercise to increase my sideways range with my elbow close to my body. I am too short to be trying to change it from the top of the door to the side of the door. That is my only complaint
